INSTRUCTIONS FOR VENDOR DIRECT DEPOSIT AUTHORIZATION

SECTION 1: Select the appropriate transaction type(s).

SECTION 2: Provide the Social Security Number or Employer Identification Number (EIN). Provide the Phone Number, Name, Email Address and Mailing Address.

SECTION 3: The person authorizing the Direct Deposit setup MUST sign and date the form.

SECTION 4: Ask your financial institution for your account information PRIOR to filling out the application.
