Foster Youth to Independence Public Notice

Public Notice
The Jacksonville Housing Authority is accepting referrals
from the following Public Child Welfare Agencies (PCWA):
Family Support Services of North Florida, Inc. and
Changing Homelessness for the below-listed initiative:
Foster Youth to Independence (FYI) Vouchers
Youth Eligibility - All of the following conditions listed
below must be met in order to qualify:
1. Youth that are at least 18 years of age and not more
than 24 years of age:
2. Youth that has left foster care, or will leave foster care
within 90 days, in accordance with a transition plan
described in section 475(5)(H) of the Social Security
Act: and
3. Youth that is homeless or is at risk of becoming
homeless at age 16 or older.
Open Enrollment for these Foster Youth to Independence
Vouchers (FYI) will remain open until all slots are filled. All
preliminary applications will be received by: FSS of N. Florida
and Changing Homelessness at: fyivouchers@fssnf.org
Criteria: The ONLY population eligible to be assisted with
funding are youth certified by the named PCWA. Eligibility is
not determined by the Jacksonville Housing Authority (JHA);
therefore, JHA will not provide the application or eligibility.
